ChoiceMMed Meds is a Patient and Caregiver Medication Reminder app for ChoiceMMed Cloud Patient Monitoring System. It designed to be used by patients and caregivers enrolled in managed care plans. It provides the following functionality:* Configure and review Medication Reminders using MyMed Reminder bracelet* Medication Adherence Analysis* Data synchronization with the managed ChoiceMMed Cloud platform.The app is not available for individuals not enrolled in managed care plans. Please contact your managed care provider for the access code to use the app.
